【】目的:探讨薄层液基细胞学(TCT)联合高危型人乳头瘤病毒(HPV)E6/E7信使核糖核酸(mRNA)检测在宫颈癌与癌前病变筛查中的应用效果。方法:选取2023年12月~2024年11月期间菏泽市牡丹人民医院接受高危型HPV-脱氧核糖核酸(DNA)筛查结果异常的患者140例,所有患者均采用TCT检测及TCT联合高危型HPV E6/E7mRNA检测。将病理活检结果作为“金标准”,比较TCT及TCT联合高危型HPV E6/E7mRNA对于宫颈癌与癌前病变的诊断效能(敏感度、特异度、准确度)。结果:140例宫颈病变患者经病理活检确诊为宫颈癌与癌前病变35例(25.00%),其中TCT诊断真阳性27例,TCT联合高危型HPV E6/E7mRNA诊断真阳性34例。TCT联合高危型HPV E6/E7mRNA检测对于宫颈癌与癌前病变的诊断敏感度、特异度、准确度均高于TCT检测,差异有统计学意义(P<0.05)。结论:TCT联合高危型HPV E6/E7mRNA检测能够提高宫颈癌与癌前病变的诊断效能,为后续临床诊疗的及时开展提供了有力的保障。 |

Objective: Exploring the application effect of thin-layer liquid based cytology (TCT) combined with high-risk human papillomavirus (HPV) E6/E7 messenger RNA (mRNA) detection in the screening of cervical cancer and precancerous lesions. Method:140 patients with abnormal results in high-risk HPV deoxyribonucleic acid (DNA) screening at Mudan People"s Hospital in Heze City from December 2023 to November 2024 were selected. All patients underwent TCT testing and TCT combined with high-risk HPV E6/E7 mRNA testing. Using pathological biopsy results as the "gold standard", compare the diagnostic efficacy (sensitivity, specificity, accuracy) of TCT and TCT combined with high-risk HPV E6/E7 mRNA for cervical cancer and precancerous lesions.Result: 140 patients with cervical lesions were diagnosed with cervical cancer and precancerous lesions through pathological biopsy in 35 cases (25.00%), including 27 true positive cases diagnosed by TCT and 34 true positive cases diagnosed by TCT combined with high-risk HPV E6/E7 mRNA. The sensitivity, specificity, and accuracy of TCT combined with high-risk HPV E6/E7 mRNA testing for the diagnosis of cervical cancer and precancerous lesions are higher than those of TCT testing, and the difference is statistically significant (P<0.05). Conclusion: The combination of TCT and high-risk HPV E6/E7 mRNA testing can improve the diagnostic efficiency of cervical cancer and precancerous lesions, providing strong guarantees for timely clinical diagnosis and treatment. |
