      目的:分析薄层液基细胞学(Thinprep cytologic test,TCT)结合鳞状细胞癌抗原(Squamous cell carcinoma antigen,SCC-Ag)、糖类抗原125(Carbohydrate antigen 125,CA125)以及人乳头瘤病毒脱氧核糖核酸(Human papilloma virus DNA,HPV DNA)检测在宫颈癌诊断中的应用表现及准确性。方法:选取利津县妇幼保健院2022年12月至2023年12月就诊的宫颈病变患者80例为研究对象,另选取同期入院的非宫颈病变患者30例作为本次研究对象,将其依次纳为研究组与对照组。对所选研究对象均展开TCT结合SCC-Ag、CA125及HPV DNA检测。并以宫颈病理活检结果为“金标准”,对比研究对象的检测结果,以及各指标在宫颈癌诊断中单项检测与联合检测的应用表现。结果:病理结果显示,80例宫颈癌病变患者中,宫颈癌60例,宫颈上皮内瘤变20例,宫颈上皮内瘤变组与宫颈癌组患者的SCC-Ag表达水平分别为(1.58±0.27)ng/ml、(4.84±2.28)ng/ml,CA125表达水平分别为(38.48±5.44)IU/ml、(46.19±6.24)IU/ml,均高于对照组健康妇女(0.45±0.23)ng/ml、(21.66±7.13)IU/ml(P<0.05);在诊断效能上,TCT、SCC-Ag、CA125与HPV DNA联合检测的敏感性、特异性、准确性、阳性预测值以及阴性预测值表现均优于各指标单项检测(P<0.05)。结论:将TCT与肿瘤标志物SCC-Ag、CA125以及HPV DNA检测联合应用于临床诊断中,有助于提高宫颈癌的阳性检出率,可加强诊断结果的敏感性与准确性,对推进宫颈癌筛查、诊治工作开展具有重要意义。

      Objective: To analyze the application and accuracy of thinprep cytology test (TCT) combined with squamous cell carcinoma antigen (SCC-Ag), carbohydrate antigen 125 (CA125), and human papillomavirus DNA (HPV DNA) detection in the diagnosis of cervical cancer. Method: 80 patients with cervical lesions who visited Lijin County Maternal and Child Health Hospital from December 2022 to December 2023 were selected as the research subjects, and another 30 patients with non cervical lesions who were admitted during the same period were selected as the research subjects. They were sequentially included in the study group and the control group. Perform TCT combined with SCC-Ag, CA125, and HPV DNA detection on all selected research subjects. And the results of cervical pathological biopsy are used as the "gold standard" to compare the detection results of the research subjects, as well as the application performance of various indicators in the diagnosis of cervical cancer through single and combined testing. Result: Pathological results showed that among 80 patients with cervical cancer lesions, there were 60 cases of cervical cancer and 20 cases of cervical intraepithelial neoplasia. The expression levels of SCC-Ag in the cervical intraepithelial neoplasia group and cervical cancer group were (1.58 ± 0.27) ng/ml and (4.84 ± 2.28) ng/ml, respectively. The expression levels of CA125 were (38.48 ± 5.44) IU/ml and (46.19 ± 6.24) IU/ml, respectively, which were higher than those in the control group of healthy women (0.45 ± 0.23) ng/ml and (21.66 ± 7.13) IU/ml (P<0.05); In terms of diagnostic efficacy, the sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of the combined detection of TCT, SCC-Ag, CA125, and HPV DNA were all better than those of individual detection indicators (P<0.05).Conclusion: The combined application of TCT with tumor markers SCC-Ag, CA125, and HPV DNA detection in clinical diagnosis can help improve the positive detection rate of cervical cancer, enhance the sensitivity and accuracy of diagnostic results, and is of great significance for promoting cervical cancer screening and diagnosis.
